How can I get a free medical ID?
As a 501(c)(3) nonprofit organization, MedicAlert Foundation’s goal is to make the protection of MedicAlert available to as many people as possible. We recognize that there are people living with a chronic or life-threatening medical condition who are also also facing financial hardship.
Thanks to the generosity of our donors, you can apply for a grant from our sponsored member program. MedicAlert’s sponsored program offers a free MedicAlert ID and 1 year Safe & Found membership for people who qualify based on financial need.
To apply for a free medical ID and one year of protection plan coverage, follow these three steps.
Provide proof of income - choose one document from the options below:
From employer:
- Recent pay stub
- Proof of Income letter
Tax documents:
- Last year’s tax return — W2 or 1040
- Wage & Tax Statement
Documentation of unearned income:
- Social Security Proof of Income letter
- Proof of Income letter from any governmental benefits program
- Unemployment benefits statement
- Workers Comp benefits statement
- Court ordered agreements
- Any type of pension or annuity statement
- Bank statement
- 1099 form
Complete the enrollment form, including a statement of need
- Download the form here
- Type directly into the document (the form is an editable PDF that you can populate by clicking the fields and typing your answers)
- Important: Once complete, save the form document to your device or computer where it’s easy to access for printing, emailing, or faxing to MedicAlert
Submit both the proof of income and completed enrollment form in 1 of 3 ways:

To be considered for the sponsored program, you must submit a complete application that includes both proof of income and the completed enrollment form. We’re sorry, we are unable to process incomplete applications.
Once granted a sponsorship, you will be covered by a MedicAlert protection plan for 1 year. At the end of the one year term, all sponsored individuals must reapply to continue receiving services.
Because the number of sponsored memberships is dependent on funding we receive through donations, MedicAlert cannot guarantee sponsorship and reserves the right to deny sponsorship at any time.
